China Visa Application Checklist: Required Documents & Personal Information Guide

Applying for a Chinese visa requires more than just a passport and photo. Applicants may also need to provide information about their employment, education, family, previous Chinese visas, travel history, and other personal background.
To make the process easier, we have prepared this China visa application checklist to help applicants organize their documents and information before submission.
Step 1: Complete Your Personal Information
Complete the personal information Word file provided to you.
If multiple family members are applying, please complete the required information for each applicant separately.
Step 2: Gather the Required Documents
Please collect all required documents listed below.
Step 3: Send Everything for Review
Once all documents and information are ready, email the complete package to us for review.
If anything is missing or additional information is required, we will let you know before proceeding with the application.
Required Documents for a Chinese Visa Application
Please prepare the following:
- Original Passport
Your passport should generally be valid for at least six months and contain sufficient blank visa pages. - Copy of the Passport Photo/Biographical Page
This is the page containing your photograph, name, passport number, date of birth, and other personal information. - Copy of a Blank Visa Page
- Old Passport
If an old passport contains a still-valid Chinese visa, please provide the old passport as well. - Copy of Your Most Recent Chinese Visa and Relevant Old Passport Biographical Page
If you have previously received a Chinese visa, please provide a copy of your most recent Chinese visa and the biographical page of the passport containing that visa. - Copy of Your Driver’s License
Please provide a clear copy or scan. - One Digital Passport-Style Photo
Please provide a recent color passport-style photograph with a white background in digital format.
Additional documents may be required depending on the applicant’s nationality, visa category, personal circumstances, and current requirements of the relevant Chinese Embassy, Consulate, or visa-processing authority.

Please provide:
- City

2. Employment History for the Last Five Years
Please list your employment history covering the last five years.
For each employer, provide:
- Company name
- Company address
- Job title
- Supervisor’s name
- Supervisor’s title
- Supervisor’s phone number
- Dates of employment
Please list each employer separately if you have worked for multiple companies.
3. College or University Education
Please provide:
- College/university name
- Start month and year
- End month and year
- Major/field of study
4. Place of Birth
Please provide your:
- City of birth
- State/Province of birth
5. Do You Hold Any Other Nationality?
Yes / No
If yes, please provide:
- Country
- Passport number or national ID number associated with the other nationality
6. Do You Have Permanent Residence in Another Country or Region?
Yes / No
If yes, please specify the country or region.

If yes, please provide information for each trip:

If you traveled to China multiple times, please list each trip separately.
20. Does Your Passport Have at Least Two Blank Pages Remaining?

Final China Visa Document Checklist
Before sending your application materials for review, make sure that:
- Your passport has sufficient remaining validity.
- Your passport contains sufficient blank visa pages.
- Your passport biographical page is clear and readable.
- Copies of previous Chinese visas have been provided, if applicable.
- Your digital passport-style photo is ready.
- Your five-year employment history is complete.
- Your college/university information is complete.
- Your parents’, spouse’s, and children’s information is complete where applicable.
- Your recent China travel history is complete.
- Your international travel history for the past 12 months is complete.
- Your phone number, email address, and emergency contact information are included.
- Any applicable military service information has been fully disclosed.
Prepare Early for Your China Visa Application
Chinese visa applications can require a significant amount of personal and travel information. Applicants who have held previous Chinese visas, used multiple passports, traveled frequently to China, worked for several employers, or have other relevant background information should allow additional time to organize their documents.
If you are planning a trip to China, start preparing your visa application as early as possible. Allow enough time for document review, corrections, supplemental documentation, and visa processing before your intended departure date.
